JOB TITLE: Medical Records Coordinator 

SOURCE OF SUPERVISION: Administrator 

COLLABORATES WITH AND REFERS INQUIRES TO: Medical Records Consultant 

DUTIES PERFORMED: 

· Assemble and audit patient’s records. Document the deficiencies for physician, nursing, and other discipline responsible for entries in the medical record. 

· Advise the person responsible for documentation of the record that there are deficiencies for completion. 

· Mail a form to the physician for completion, such as the discharge summary.

· Document each discharge in the discharge analysis/discharge log.

· Document the patient index with the appropriate information and file with the discharge patient cards. 

· Audit thinning/overflow, get it completed and file. The overflow files must be maintained in the order of the discharge patient’s record. 

· Answer correspondence regarding the release of information, following the instructions of the manual of “Release of Health Information.” 

· Accept and answer subpoenas to the procedure manual, “Subpoena Procedures.” 

· Have experience with electronic health records software Point Click Care and be the lead for the facility. 

· Assist in other areas as directed by the Administrator, i.e. QA, QAA, QAPI, Disaster Preparedness, LSC, etc.

QUALIFICATIONS: 

• High school diploma or equivalent qualification. 
• A minimum of 3 years’ experience in a similar role. 
• Advanced understanding of medical terminology and administration processes. 
• Proficient in information management programs and MS Office. 
• Proficient in Point Click Care.
• Outstanding communication and interpersonal abilities. 
• Strong attention to detail with excellent organizational skills. 
• Typing skills.
